Infusion Consultants partners with specialty pharmacies to deliver reliable, well-coordinated home infusion nursing services supported by clear communication, timely documentation, and consistent clinical standards. Our processes are designed to simplify referral intake, ensure continuity of care, and maintain alignment between pharmacy, provider, nurse, and patient.

Referral Intake & Scheduling

Referrals are accepted through AlayaCare or securely via email.

Each referral is reviewed promptly. When nursing coverage is available in the service area, our team confirms clinical appropriateness and provides firm nurse availability dates to support timely start of care.

Care Coordination & Patient Onboarding

Once scheduling is confirmed and both the pharmacy and Infusion Consultants agree to proceed:
- Orders and required documentation are received and reviewed
- A welcome call is completed with the patient to confirm visit details, medication arrival, and expectations
- The assigned RN contacts the patient within 24 hours to introduce themselves and review the upcoming visit
This proactive communication helps reduce delays and ensures a smooth start of care.

Ongoing Communication & Alignment

We prioritize clear, consistent communication throughout the course of therapy. Our team ensures:
- Timely updates to pharmacy partners regarding patient status or schedule changes
- Prompt communication of any nurse-reported concerns, patient issues, or clinical changes
- Alignment with the prescribing provider as needed to support patient safety and
continuity

Documentation & Compliance

All nursing documentation is completed electronically in AlayaCare
- Documentation is submitted timely, complete, and in accordance with pharmacy
standards
- Additional forms or reporting requirements may be completed upon pharmacy request

Quality Assurance & Feedback

To support continuous improvement:


- Patient and nurse feedback surveys are completed following the first cycle of care
- Ongoing surveys are conducted twice annually
- Feedback is reviewed to identify opportunities for process and service enhancements
